PART 1. THE BASICS




1. Write a brief description about yourself. (First name, gender, age, civil status & any other basic facts you think is important)

2. Do you think you could be 100% honest in answering these questions?

3. Rate you confidence level from 0 - 10.

4. Talk about your day-to-day life & how you feel about it. (Sample Keywords: HAPPY, BORED, SATISFIED, EXCITED, DEPRESSED, RESIGNED, etc..)

5. What were your hobbies as a child? Now as an adult, do you still have these same interests?

6. Do you easily get bored? What do you usually do whenever you get bored?

7. Describe your childhood. (Sample Keywords: HAPPY, MODERATELY HAPPY, TRAUMATIC)

8. How were you disciplined by your parents? What were the things that you feared the most?

9. How were you treated as a child? Were you raised by nannies, relatives or your parents?

10. What are your earliest memories? Name at least 3 animate or inanimate objects, sights, sounds or smells that reminds you of your childhood.

11. Are you still in touch with your family & relatives? Do you think your family would actually like you as an individual if you aren't of their blood? Do you dislike someone in your family?

12. If there are things you'd want to change about how you were raised, what would that be?

13. Cite any & every "FAVORITES" you remember in your childhood.

14. TRUE or FALSE: "The early childhood of a person's life is the biggest determining factor        on how he would turn up to be as an adult."
